Dr. SArah Ellefson, PT, DPT, OCS

Founder and director of Altius- Avon, Sarah specializes in orthopedic rehabilitation of the spine and degenerative hip, women’s health across the lifespan, chronic pain management, and return-to-performance training. Her clinical approach integrates biomechanics, strength science, and metabolic health to deliver personalized, evidence-based care designed for long-term resilience—not just short-term recovery. Beyond her clinical practice, Sarah is a content creator and on-screen instructor for Oxefit, Inc., where she develops rehabilitation-informed programming that transforms complex movement science into intelligent, accessible digital training. She is also Co-Founder of the Vail Institute of Pilates and has served for more than a decade as an Adjunct Professor of Anatomy & Physiology at Colorado Mountain College. Whether in the clinic, the classroom, or on screen, her mission is consistent: to elevate the standard of healthcare through evidence-based practice, clinical expertise, and intentional design.

Sarah has a deep appreciation for the outdoors, finding joy in the mountains, desert and ocean. She is an avid trail runner, mountain biker, and skier, and recently took up surfing—embracing both its challenge and (mostly) humility. She’s always up for multi-day backcountry adventures, learning something new and connecting around a campfire with her friends, her husband Sylvan, and their two sons, Ryder and Aksel.
